"Hallucifer" is the hallucination of Lucifer that plagues Sam after his mental wall was destroyed by Castiel.

7.01 Meet the New Boss
Soon after Castiel declared himself God, Sam begins hallucinating. At first, it's simply hearing Lucifer call his name and then hallucinating meat hooks and chains hanging from Bobby's basement, including one that reaches down and strangles him in the kitchen, until finally he sees "Lucifer," who tells Sam that his escape from the Cage was all an elaborate torture, and he is still locked away in Hell.
7.02 Hello, Cruel World
Lucifer continues to taunt Sam on how this world isn't real. He points out that he made this world, because it was the most believable for him. Sam later confronts Lucifer and asks him why he doesn't end this dream world; Lucifer responds that he not only enjoys it, but points out how it only ends when Sam can't take it anymore. Lucifer later changes into Dean and tricks Sam into leaving Bobby's house and driving to an abandoned warehouse, making it appear as an office where a team of leviathans nested.
Once at the destination, he reveals himself to prove he can control Sam's "dream." Lucifer is proven to be a hallucination when Dean explains to Sam how to tell the difference between hallucinations and reality. Sam discovers that squeezing his injured palm, which the real Dean sewed up, can get rid of him for a time. Despite being proven as a figment of Sam's imagination, Lucifer informs Sam that he's not going anywhere.
7.03 The Girl Next Door
At Rufus's Cabin, Sam begins hearing Lucifer for the first time in weeks, taunting him that he is still in the Cage and never left, but he just shrugs it off.
7.04 Defending Your Life
Sam continues hearing Lucifer's voice, continuing to deal with it by focusing his mind on pain.
7.06 Slash Fiction
While sitting at a diner, preparing to slaughter everyone, Leviathan!Sam and Leviathan!Dean lament about having to be the Winchester's doppelgängers, with Levi Sam telling Levi Dean that it was "nothing but Satan-vision on the inside." The resulting hallucinations did not seem to affect the Leviathan, and appeared to be more of a nuisance rather than a hindrance.
7.09 How to Win Friends and Influence Monsters
Sam and Bobby talk about how things are going. Bobby asks about Sam's mental state and Sam comments that he has it under control while touching his scar, believing that he thinks that seeing Lucifer, as unfun as it is, was the best case scenario compared to what could've happened. Bobby doesn't believe this but lets it go, noting how deep Sam always was.
7.10 Death's Door
After Dean storms off, Sam is seen using his scar to keep Lucifer at bay while dealing with the state of Bobby Singer after Bobby is shot in the head by the Leviathan leader Dick Roman.
7.15 Repo Man
Lucifer begins taunting Sam in the motel room, eventually forcing Sam to press hard on his wounded hand until Lucifer flickers away. Later at a crime scene Lucifer tries to help Sam remember a detective's name from their prior case, only to be continually ignored. At Jeffrey's apartment, the hallucination gets annoyed by Sam's obvious choice of words, saying that it chooses to disappear on its own when Sam attempts to squeeze his injured palm.
Lucifer returns once again, trying to tempt a recalcitrant Sam into acknowledging his existence while Sam is researching at the local library. While Sam tries to ignore the hallucination, he is eventually forced to talk with his hallucination when Lucifer offers to "help" Sam with the current case, helping Sam realize that they were dealing with Jeffrey, the meatsuit of the demon they had been hunting last time, trying to summon the demon back to possess him. He even motivates Sam to squeeze the truth out of Nora to get the full story until they use a tracking spell to find where Jeffrey was holding her son.
After the case is done, Lucifer appears before Sam once again, forcing him awake. When Sam tries to send him away, the hallucination tells Sam that since he acknowledged his presence, he let him in and nothing Sam can do will get rid of him. Snapping its finger, the hallucination forces Sam to see the entire room engulfed in flames and Lucifer laughs.
7.16 Out with the Old
Although Hallucifer does not appear, Sam tells Dean that he can't get rid of him anymore after letting Hallucifer in and that he is forcing Sam to stay awake with unending noise, including singing "Stairway to Heaven" over 50 times in a row at one point. Throughout the episode, Sam also tries to keep himself awake with increased caffeine consumption and loud music to focus on hunting and driving.
7.17 The Born-Again Identity
Lucifer's torture becomes so bad Sam is put in a mental institution. While there, Lucifer continues to torture him, linking it to when they were in the pit, making him hallucinate maggots in his food and throwing fire crackers at him to keep him up. Eventually, after being forced to undergo electroshock treatment by a demon, it becomes too much and Sam snaps, the remnants of the wall in his mind got crushed to dust by whatever's going on inside his mind. Even when Castiel made his way to Sam, he couldn't be healed as it reached the point where all people that Sam sees are Lucifer. In a last act to redeem himself for his previous actions, Castiel decides to shift the mental torture to get Sam back on his feet and transfers the hallucinations from Sam's mind into his own. Lucifer, uncaring about the change, just calmly says, "Hello brother" and presumably goes on to torture Castiel as he did to Sam.
7.21 Reading Is Fundamental
Sam asks Castiel if he sees Hallucifer, but Cas tells him that he only saw him at first, a projection and an aftertaste of Sam's experience, but he no longer sees him, but began noticing everything else. It implies that Hallucifer is no longer torturing Castiel's mind and is gone for good.
Trivia
- "Hallucifer" is a fan-created nickname for Sam's hallucinations of Lucifer during season seven. Occasionally fans will spell it "Halucifer." It originated in the LJ community from ontd_spnparty's 7x01 Party Post.
- A subconscious manifestation of Lucifer also appeared to Jack Kline in Season 14. Fans dubbed this version "Jackifer."
