Mildred witnesses the death of the manager of the retirement home by what she describes as a "spirit woman". Sam and Dean determine this to be a banshee—the malevolent kind, who prey on the victim’s brain and tend to search for vulnerable people. It is being hunted by a hunter, and Men of Letters legacy, called Eileen Leahy.

As she is ill, Sam and Dean suspect Mildred may be a target and she agrees to act as bait. While they prepare, Mildred and Dean talk about life and she tells him, "Follow your heart. You do that, all the rest just figures itself out."

Dean then hears the banshee’s screech. The spirit is attacking the others, while Dean starts to bash his head into the wall. Eileen gestures to Mildred to use the sigil to trap the banshee, allowing Eileen to finish it off with her golden blade.
