Synopsis
Around St. Valentine's Day, people begin killing themselves in odd ways, such as a couple who ate each other to death. Sam and Dean visit the coroner to check out the bodies and Sam sees a suspicious man walking past him in the hallway. As the brothers examine the hearts of the victims, Sam notices what he thinks to be an Enochian Sigil on both hearts. They show them to Castiel and he agrees, saying that the sigils are a sign that the two people were intended to mate, and thinks that it is the work of a cupid (a lower level angel, cherub 3rd class) gone rogue. Castiel takes the brothers to a restaurant where Cupid is sure to be. As the waitress brings over the food that Sam and Dean have ordered, Dean doesn't feel like eating his burger and Castiel takes it, intending to eat it himself. Cupid arrives at the restaurant as wind, which makes many of the couples in the restaurant start to get lovey-dovey. Castiel traps Cupid in the back room of the restaurant and forces him to show himself.
Cupid reveals himself to be a naked man and goes around giving Dean, Castiel and Sam big hugs, which include lifting them up off the floor. Castiel adds that this is the Cupids way of shaking hands. As they interrogate Cupid, he says that it isn't his fault his charges are slaughtering each other, that he's just following orders from Heaven. He says that some couples, like John and Mary Winchester were just meant to be together and that Heaven uses Cupids to insure that Sam and Dean were born. This upsets Dean and he punches the Cupid, causing it to get upset also and leave.
Another person dies and Sam goes to check it out at the coroner's office and his way out he sees the Demon from before carrying a briefcase, follows him into an alley and attacks him with Ruby's Knife. The Demon gets away, but Sam gets its briefcase. At the motel, Sam and Dean open the briefcase which turns out to be a burst of white light. Castiel shows up eating a hamburger and tells them that the light was a human soul, which is most likely intended to use as a food source. He believes that the town is suffering from Famine, one of the Four Horsemen of the Apocalypse, who brings out the things people really starve for in themselves: sex, attention, drugs, love... everybody is starving for something. Castiel also believes that he himself has been infected by his vessel's fondness for red meat.
Famine, a very old frail looking man in a wheelchair, arrives at a restaurant in a black Cadillac Escalade with Lucifer's demons flanking him. As soon as he enters the restaurant, the customers begin giving in to their cravings, many of them fatally. The demon that Sam attacked also arrives toting Sam's room key and gives it to Famine. After telling the starving Famine that he has lost the human soul to Sam, Famine consumes the demon instead. Dean wants to go after Famine and cut off his power by cutting the ring off of his finger, much like with War, but Sam says that he cannot go. He thinks he's been infected and is craving Demon Blood. Dean handcuffs Sam to a pipe in the bathroom and Cas pushes a dresser in front of the door to keep him inside when they're out hunting Famine. Cas and Dean go to the coroner's office and find that the coroner had been infected and, after 20 years of sobriety, drank himself to death. Cas notices the demons have not yet harvested his soul and they stake out the place from the Impala, while Castiel eats hamburgers the entire time.
While Dean and Castiel are on stakeout, demons arrive at the motel for Sam. Sam attacks the demons and drinks the blood of one of them, and knocks the other unconscious, intending to drink his next. After following a demon back to Biggerson's Restaurant where Famine is, Cas goes in, intending to cut off Famine's ring but doesn’t come back out. Dean goes in and finds Cas eating raw meat on the floor and gets captured by demons. Famine wonders why Dean has not been infected by him and touches his chest, concluding that there is nothing inside of him -- that Dean is dead inside.
Sam busts into the restaurant and Famine stops his demons from capturing him. Famine tells Sam that he sent the demons earlier as a snack, that Sam won’t die from drinking too much blood, he’s the exception and that is exactly what Lucifer wanted. Famine offers his demons to Sam, but he refuses and exorcises all of the demons in the room at once; however, Famine consumes them instead of letting them get sent back to Hell. Sam attempts to exorcise Famine, but it doesn’t work, so he decides to exorcise all the demons that Famine has eaten, which seemingly destroys the horseman.
Dean and Castiel lock Sam up in Bobby's Panic Room, and Castiel tells Dean that it isn’t Sam inside, that he just needs to detox. As he goes outside for some air, Dean stops drinking his whiskey and begins to tear up. He prays, asking for Heaven’s help, and begins to cry.

A reference to quintessential romantic comedy When Harry Met Sally.
In the 1981 movie Clash of the Titans, based on the heroic stories of Greek mythology, Zeus instructs Poseidon to release the sea monster the Kraken, with the line, "Unleash the Kraken!" The phrase passed into popular culture. At the time the episode aired, it was featured in the ads for the 2010 remake of the film.
