Synopsis
Sam has been awake for five days, tormented by Hallucifer. He tries in vain to use drugs to pass out and in his sleep-deprived state, is hit by a car. He winds up in the locked psychiatric ward at Northern Indiana State Hospital, being treated for a broken rib, lacerations and what the doctors call a psychotic episode. The doctors have administered several sedatives but Sam remains conscious.
In the ward, Hallucifer plays cat's cradle and reminds Sam that he didn't need to sleep when he was soulless. Dean arrives and tells Sam he'll find him help. Sam doesn't think he'll be able to find anything, saying they always knew putting his soul back in would end badly. Dean leaves.
At Rufus's Cabin, Dean calls some of Bobby's contacts but is unable to find help. He puts the journal on the table and goes to the fridge for a beer. The journal falls to the ground by itself and a card for Mackey's Taxidermy falls out. Dean calls and leaves a message.
Hallucifer comments that he and Sam being in a locked ward together is just like the Cage. Sam is eating a sandwich when he discovers it is full of maggots. He drops it and backs away while another patient, Marin, watches from his door before leaving.
At the cabin, Dean is looking for help online when he receives a call from Mackey. Mackey tells him about a healer named Emmanuel in Colorado. He claims that Emmanuel passed all the monster tests and healed Mackey's damaged eye with one touch.
Dean goes to Emmanuel's house in Colorado. He sees Emmanuel's wife Daphne tied up and fights the demon pretending to be Emmanuel. He reminds the demon that Crowley ordered demons to stay clear of Dean and Sam, but the demon says that because Dean hasn't killed Dick Roman, Emmanuel is more important. Dean kills the demon with Ruby's knife. Just then he sees Emmanuel, who is the spitting image of Castiel. Emmanuel doesn't recognise Dean, has no knowledge of demons and no memory of being an angel.
Hallucifer continues to torment Sam, lighting firecrackers in the ward. An orderly, Marcus, brings Sam his meal. Sam asks Marcus about Marin and he tells him she wasn't brought to the psychiatric ward because of an accident.
Dean and Emmanuel talk in the car on the way to Indiana. Emmanuel tells him that Daphne found him by a river, when he wandered into her path "drenched, confused and unclothed", with no memory. Daphne claimed God wanted her to find him. Dean mentions the possibility that Emmanuel was a bad guy, but Emmanuel says he doesn't feel like a bad person.
Marin brings Sam a stolen chocolate bar. She tells him that she hears her dead brother's voice. He wants her to kill herself to be with him, and if she doesn't, he'll do it for her.
Emmanuel tells Dean he can cure illnesses of a spiritual origin and should be able to help Sam. Dean goes on to tell him that he is unable to forgive his friend Cas, who betrayed him and caused Sam's illness. Emmanuel still has no memory and comments that Cas is an odd name.
Dean is confronted by demons at a convenience store. He kills one and another is killed by Meg. She is there because of rumours about a wandering healer. She has an ongoing feud with Crowley and doesn't want him to know about Cas. Although she wants to use Cas as a weapon, she agrees not to tell him his true identity yet. The three of them leave together.
Sam and Marin talk about her dead brother. She tells him how she feels cold before he talks to her, and how one day he got angry and set the room on fire, almost killing her. She says he was cremated, but she wears a bracelet he made for her with an injured hand - it's likely he bled on it. She steals a lighter and makes a ring of salt after Hallucifer blows Sam's ring away. They stand inside the circle and Marin's brother appears. He breaks the lights and pleads with Marin not to do it. She gives Sam the bracelet and he burns it, causing her brother to go up in flames. After Marin thanks Sam and leaves, orderlies enter the ward to restrain Sam. Hallucifer talks over Dr Kadinsky's shoulder as the doc explains to Sam that they cannot give him any more medication and should try surgery instead.
