After being released into the water system when they took over Castiel, this Leviathan enters a young girl when she is drinking from a water fountain. At home, she watches television, and after seeing an ad for Dr. Sexy M.D., comes up with a plan to solve their feeding problem by consuming patients at a hospital.
Annie meets up with another Leviathan, Edgar. She complains about her vessel's small size, and lack of useful knowledge - although she has learned how babies are made. She shares her plan for taking over the hospital. At Sioux Falls General Dr. Gaines sees Annie alone in one of the hospital rooms and approaches her. Annie takes his form and then kills him.
