During the evacuation in Harlan, the sheriff comes across Constance, who quickly rips out his throat. When Dena and [[Belphegor]] find the body, Dean notices the drop in temperature and tells Belphegor they need to go. Constance appears, and recognizes Dean as having taken her home, before throwing him to the side into a dumpster. She then turns her attention to Belphegor, who tries to talk her down, but gets scratched across his hand by Constance before Dean hits her with iron and banishes her temporarily.

1.01 Pilot

While Troy Squire is driving home down the Centennial Highway, he notices Constance Weltch standing at the side of the road, he stops and Constance asks him take her home, Troy obliges. While driving, Constance begins to come on to Troy asking him if he will come home with her, he agrees. They arrive at Constance's home, an old abandoned house at the end of a road, and Constance mysteriously disappears. Troy investigates the house, but is scared off by a bird and runs back to his car. When he looks in the rearview mirror, Constance is in the back seat. Frightened he drives straight through a "Bridge Closed" sign, stopping about halfway across the bridge, where he is killed by Constance.

When Sam and Dean begin investigating the murder of Troy Squire, they find the bridge where Constance jumped to her death from and begin fighting, when suddenly Constance appears standing at the edge of bridge, and steps off. While Sam and Dean wonder what happened, the Impala suddenly comes to life, Constance has taken possession of the car and attempts to run down the Winchesters with it, but failing to do so.

Constance is destroyed after confronting her dead children.

While Sam is driving down the highway, Constance appears in the road in front of him. The car goes right through her as Sam brings it to a halt. Inside the car, Constance is sitting in the back seat, and tells Sam "Take me home," when he refuses she locks them in the car, takes control and begins driving. As Sam looks back, he sees Constance flicker, she reappears in the shotgun seat and climbs into Sam's lap, Constance begins kissing Sam as he continues to struggle, reaching for the keys. She pulls back and disappears. Constance flickers back in front of him, her hand reaching into his chest. A gunshot goes off, shattering the window and startling Constance. Dean approaches, still firing at her. She glares at him and vanishes, then reappears, and Dean keeps firing until she disappears again. Sam manages to sit up and start the car. Sam drives forward and smashes through the side of the house.

Now home, Constance picks up a large framed photograph of herself and her children, she sees Sam and Dean and throws the picture down. A bureau shoots towards the Winchester, pinning them against the car. The lights flicker; and a scared Constance looks around as water begins to pour down the staircase. She goes over, standing at the top are her children. Constance looks at them distraught, when suddenly they are behind her; they embrace her tightly and she screams, her image flickering. In a surge of energy, still screaming, Constance and the two children melt into a puddle in the floor.

14.20 Moriah

After Chuck declares "the End" and releases souls from hell, Constance is seen on the side of a road, where a man in a car stops to ask if she wants a lift.
