In this episode Sam uses a spirit board to communicate with Dean. A spirit board, commonly referred to as a Ouija board, a name trademarked by Parker Brothers, is a board that is used as a tool to contact spirits. The fingers of the participants are placed on the planchette, the triangle or heart shaped pointer that is used, which then moves around a board covered win numbers, letters and symbols in order to spell out messages.

When Dean lies in a coma, Sam uses a spirit board to contact him. Through the board, Dean's spirit is able to communicate with Sam, and tell him that he is hunting a reaper in the hospital.
3.03 Bad Day at Black Rock
Bela tells Dean that she used a spirit board to contact the people who had been killed by the Rabbit's Foot. They are focussed on its energy and help her locate it - stored in a Curse Box in John Winchester's lock-up.
